Mandibuloplasty & genioplasty

Mandibuloplasty and genioplasty are two surgical procedures performed on the face, with the aim of reshaping the contour of the mandible and chin. These interventions can be performed both for aesthetic reasons, to obtain a harmonious and balanced facial profile, and for medical reasons, in the case of congenital anomalies, trauma, or obvious asymmetries.

Mandibuloplasty focuses on shaping the mandible – the lower jaw bone – either by reducing the volume in the case of an overly prominent or wide jaw, or by accentuating the mandibular line. It is often chosen to redefine the facial contour, contributing to a finer, more proportionate appearance.

Genioplasty, on the other hand, aims to modify the shape and position of the chin. It can be an advancement procedure, when the chin is retracted, or a reduction, if it is too prominent. It is performed either by cutting and repositioning the bone (osteotomy) or by special implants, depending on the patient’s needs.

Who is the right candidate for mandibuloplasty & genioplasty on the face?

Everyone wants to have harmonious facial features.

When we look at the face, harmony is determined by the position of the eyes, nose, mouth, chin, and mandible. These elements form a triangle with the tip down (the base of the triangle is the line that connects the eyes).

When we age, or if the mandible is wide or the chin is small, we get the impression that the triangle has its base facing down, at the level of the mandible, and this aspect is neither youthful nor aesthetically pleasing.

If the cause of the unaesthetic appearance of the face is a wide mandible or a chin that is too small or too large, then the patient is a candidate for V-line surgery (it is called V-line because it tries to obtain an appearance of the mandible and chin in the shape of the letter “V”).

V-shape surgery has become popular in recent years. This procedure addresses the contour of the mandible, chin, or the bone that contours the cheek – the zygoma.

These aesthetic procedures can be combined with orthodontic surgery procedures that address the correct alignment of the teeth.

How is V-shape surgery performed?

V-shape surgery is performed under general anesthesia. For mandibular reduction, the incisions are made inside the mouth between the lower teeth and the cheek, then the bone is reduced at the level of the mandibular margin as much as was established preoperatively. Through the same incisions, the masseter muscle can also be reduced, which, if hypertrophied, contributes to the exacerbation of the angle of the mandible and the widening of the lower part of the face. In the next step, the bone at the level of the chin is remodeled. The chin can be narrowed, reduced, or projected forward as needed. For projection, synthetic facial implants can be used. The bone called the zygoma at the level of the cheek can also be shaped; most often, it requires impaction because it is too wide in relation to the features of the face.

Preoperatively, Dr. Olimpiu Hârceagă will take measurements on a film of the facial bones (computed tomography). Remodeling the bones will redefine the oval of the face in a harmonious way.
