In others, the cause is trauma to the nose. This can happen, for example, following a traffic accident or if you practice a contact sport. Regardless of its cause, septal deviation can worsen as we age.[1]

Symptoms

Sometimes, symptoms of septal deviation may go unnoticed. When they occur, the most common are breathing difficulties and inflammation of the nasal mucosa; they are often confused with those of a respiratory infection. Other signs and symptoms that may be associated with septal deviation are:

Nasal congestion;

Frequent sinus infections, resistant to medication;

Nosebleeds;

Formation of nasal crusts, especially in the larger nostril;

Noisy breathing during sleep;

Rhinorrhea;

Headaches or facial pain;

Tendency to sleep on the side opposite to the blocked nasal passage;

Sleep apnea (in severe cases).[2]

Complications

Sometimes, due to nasal obstruction that occurs as a result of septal deviation, complications such as dry mouth (because you tend to sleep with your mouth open) or a sensation of pressure or congestion in the nasal passages may appear. Sleep quality may also be affected.[1]

Treatment Options

The main method that can be used to correct septal deviation consists of a surgical intervention called septoplasty. This may or may not be combined with a rhinoplasty.

Septoplasty

One of the most frequently performed procedures in otolaryngology (ENT) and plastic surgery, septoplasty is performed to achieve a straight, properly centered septum, so that airflow is no longer obstructed.

Most often, it is performed under general anesthesia, but it can also be done under local anesthesia.

What to expect:

Before surgery

First, a specialist will take a medical history and examine you to determine whether or not you are a good candidate for septal deviation surgery. They will also discuss with you the reasons behind your decision and the expectations you have regarding the results of the intervention. Photographs of the nose will also be taken from various angles. After the intervention, these will help you compare the differences, but they will also be useful to the doctor for planning the procedure.

If you take certain medications, such as anticoagulants, for example, you will be asked to stop treatment at least two weeks before surgery. If the intervention will be performed under general anesthesia, you will be asked not to eat or drink anything the night before.[3]

During surgery

The intervention can last between 30 and 90 minutes. Various techniques can be used, but a typical procedure involves making an incision in the nasal mucosa, removing excess bone or cartilage, correctly repositioning the nasal septum and mucous membrane.[3]

After surgery

Most likely, you will be able to go home the same day. Your nose will be swollen and painful. For a few days, you may have slight bleeding. Also, swelling and bruising will appear around the eyes. Cold compresses can help you feel more comfortable. The doctor may recommend painkillers, but you will still be asked not to take anticoagulants.

It is also recommended to avoid activities or exercises that may increase blood pressure for at least a few weeks after surgery. For faster recovery:

Sleep with your head elevated;

Do not blow your nose for at least 3 days after surgery;

Wear shirts or blouses that button up to avoid pulling them over your head.[3]

Rhinoseptoplasty

In some cases, septoplasty is combined with rhinoplasty, an intervention aimed at correcting the shape and/or size of the nose. In this case, we speak of rhinoseptoplasty. Dr. Olimpiu Hârceagă uses piezosurgery for such interventions.

Instead of files and chisels of different sizes, a device that generates vibrations with a very high frequency and small amplitude is used for cutting and reshaping bones. This device has a metal tip that, in contact with the bone, causes it to be cut. Unlike the classic technique, bone cutting through piezosurgery is much finer and more controlled, performed under visual control, soft tissues are not traumatized, and recovery is faster.

Symptomatic treatment

When symptoms are not severe or when, for various reasons, septoplasty cannot be performed, medication treatment may be recommended. In this case, the following may be prescribed:

Decongestants;

Antihistamines;

Nasal sprays with corticosteroids.

These will not correct the septal deviation, but may contribute to symptom improvement.[1]

Septal deviation is a common condition. Depending on its severity, it may or may not affect your quality of life. The only method to correct the position of the nasal septum is septoplasty. Contact us to determine together whether or not you are a suitable candidate for such a procedure.

Bibliography:

“Deviated Septum – Symptoms and Causes.” Mayo Clinic, 2021.
