Historically, the first procedure of this kind was performed at the end of the 19th century, specifically in 1893, by a German plastic surgeon, Gustav Neuber, who used fat extracted from the patient’s arm to correct scars caused by osteomyelitis. Its popularity began to grow only in the 1990s, after a plastic surgeon from New York, Sydney Coleman, began publishing works describing standardized techniques for fat extraction, processing, and injection.[2]

What the procedure involves

Autologous fat transfer is performed in three stages: harvesting, processing, and injection. Before discussing each of the mentioned stages, we should add that this procedure can be performed under local or general anesthesia. The type of anesthesia to be used is usually determined based on the volume of fat to be extracted; for extracting small quantities, the use of a local anesthetic is sufficient; for larger quantities, however, general anesthesia is almost always necessary.[2]

Harvesting

The first step consists of harvesting adipose tissue through liposuction. Not only is the necessary amount of fat obtained for injection, but the donor area is also remodeled. There are various adipose tissue extraction techniques, and your surgeon can tell you which is the best and safest.

Regarding the areas from which the necessary fat is extracted, the most common are the abdomen, thighs, and knees. After examining you, your surgeon will identify the most suitable area.[3]

Processing

Once the fat has been harvested, the next stage can begin: processing and purifying it. This stage is necessary because, in addition to fat cells (adipocytes), the extracted fat also contains collagen fibers, blood, and other contaminants that can affect the success rate of the procedure.[3]

Injection

After being purified, the fat can be injected into the area where there is a deficit, using a needle or cannula. The amount of fat injected can vary from case to case and depending on the area of the body being treated.[1]

Areas where autologous fat transfer can be performed

Among the most common areas that can benefit from autologous fat transfer are:

Face

At the facial level, autologous fat transfer is used to add volume in areas where it has been lost or where expression lines exist, such as the under-eye area or nasolabial folds (those wrinkles between the nose and mouth). It can also be used to add volume to the cheeks. Additionally, it helps correct scars and improve skin texture and appearance.[2]

Dr. Olimpiu Hârceagă uses fat extracted from the lower face level for selected cases to restructure the upper level, especially the cheekbone and periocular area.

Breast area

At the breast level, autologous fat transfer is used for breast augmentation, correction of asymmetry and deformities, and even for breast reconstruction.[2]

Breast augmentation with fat transfer, for example, is considered a more natural, minimally invasive approach to enlargement, as it uses the body’s own fat cells instead of silicone implants, and the results are satisfactory and long-lasting. Among the benefits of such an intervention are:

There is no risk of displacement;

Fat is much lighter than silicone and will not cause breast sagging over time;

It can be used after implants or in combination with a light implant procedure.

Buttocks

Autologous fat transfer can also be the right procedure for those who want firmer buttocks with a more beautiful shape.[Rodriguez] Brazilian butt lift is the name of the procedure that involves injecting fat into the upper areas of the buttocks, as well as other places that could enhance their shape. The result: buttocks with a lifted, rounded appearance.

In addition to these three areas, fat transfer can also be used to improve the appearance of hands and feet, as well as to achieve a more harmonious silhouette.[2]

Advantages and disadvantages

Like any procedure, this one also has both advantages and disadvantages. Among its advantages are:

It uses the body’s own fat, not a synthetic product; often, this is available in large quantities and can be harvested relatively easily;

Natural results are obtained;

Both the transfer area and the donor area are treated simultaneously.

Among the disadvantages we will mention:

The injected tissue can be reabsorbed;

A surplus of fat is necessary for extraction and injection;

It is a time-consuming process and multiple sessions may be necessary.[1]

Autologous fat transfer has been used for decades to correct soft tissue defects in various areas of the body. As with any surgical procedure, the technique used, its execution, and the surgeon’s experience will affect the final results.
